Happy Health Information Professionals Week to our fabulous Medical Records team!  This hardworking team manages ALL the health information and Electronic Health Records at Special Tree -- making sure we have the right info where and when its needed while ensuring its accuracy, accessibility, and security.  Thanks to Judy Freels, Chantelle Miller, and Jennifer Graves for the important work they do to make sure our clients receive Fabulous Everyday Care Every Day! 

We asked Judy Freels, Health Information Coordinator for 17 years, a few questions to help us better understand her team's critical role at Special Tree.

Who is on the team and what are their roles at Special Tree?

Judy:  I process all the requests for medical records from attorneys, other health facilities, insurance companies, and clients as well as maintain and report all updates on census for Special Tree.  I also enter profiles in Matrix for new client admissions and make sure proper legal paperwork is in Matrix.   I perform monthly chart audits with other team member to maintain quality chart documentation, and review and update yearly client contract per state requirements and reimbursement.

Jennifer Graves, Health Information Assistant, Special Tree team member since 2001.  Jennifer assists in the management of the EHR and EHR updates, and provides education to our new hire Nurses.   She also conduct audits for the NeuroCare Center and our Physician Services to ensure our clients are receiving Fabulous Every Day Care Every Day.

Chantele Miller, Health Informatics Specialist, Special Tree Team Member Since 2009.   As the Health Informatics Specialist,  Chantele is responsible for facilitating communication from the clinical end users across client care departments while working collaboratively and directly with the vendors and technical experts. In addition, she providse assistance to all system users, and provides subject matter expertise on all clinical informatics systems.  She serves as a liaison between clinical department(s) and IT kas well as system software support.   Chantelle is responsible for understanding and implementing improvements, enhancements, and modifications of system software as well as, developing methods for collecting, organizing, analyzing, and reporting data in collaboration with other departments to achieve optimum use of the Electronic Health Record. She was just inducted into The National Society of Leadership and Success, is a member of the Michigan Chapter of the Healthcare Information and Management Systems Society, and is a certified as a Michigan Performance Excellence Examiner.

What was your team’s biggest accomplishment in 2018?
Revision of the Nursing Competency Day and implementing New Hire Nursing Boot Camp for incoming nurses.

What does your team hope to accomplish this year?
Judy:  Ani.Increase in documentation compliance through chart audits, and efficiency in physician order entry through New Hire Nursing Boot Camps.

What are your team’s strengths?
Judy:  As a team, we have strong project management skills and excellent teamwork and communication skills.
