Special Tree residential living provides adults with an opportunity to live in the community while still participating in a customized rehabilitation program. Residential living is a great solution for clients on both a long-term and transitional basis depending upon their individual needs.

Comprehensive Care + Services in a State-of-the-Art Neurorehabilitation Center

 

Our 15-bed facility at our NeuroCare Center South provides a perfect transition from our acute care with access to all of our campus amenities and staff.

Also onsite is our Heavlin Residence, where clients enjoy increased independence in a home specially modified to accommodate the needs of persons with disabilities while maintaining a warm, family-like home setting.

With 24-hour supervision and care from our trained, experienced staff, clients make choices to manage their participation in a wide range of activities both at Special Tree and in the community.

 
  • Clients make choices to manage their participation in a wide range of activities all with the guided supervision of trained professionals dedicated to their care.

  • The NeuroCare Campus includes a full service subacute rehabilitation unit, with 24/7 on-site nursing and respiratory teams who frequently consult with and advise our residential staff to support client needs whenever necessary.

  • All clients receive a nutrition assessment upon admission to help develop a nutrition care plan based on their individual needs. The nutrition status of each client is monitored through dietary intake, tube feeding intake, fluid intake, weights, laboratory values and input from their support system and Special Tree staff.

  • The NeuroCare Campus includes a totally comprehensive rehabilitation clinic which provides a full range of on-site therapy and treatment services. For detailed information about our rehabilitation services, click here.

  • Rehabilitation Service Technicians (RSTs) help clients with personal care and activities of daily living. Special Tree RSTs complete extensive classroom and hands-on training, and participate in continuing education to maintain skills and stay current with best practices in care, safety, and rehabilitation.

  • Your Special Tree Case Manager is the vital link between you and those involved in your care. Your Case Manager will assist you and your family with scheduling doctor appointments, transportation needs, progress reports, resolving day-to-day problems that may arise, and discharge planning.

  • Coping with a brain injury or a spinal cord injury can present unique and complex challenges. Our team of Psychologists and Social Workers help identify the social and emotional needs of clients and families, and implement plans to support adjustment to the rehabilitation process.

NeuroCare Center South

On the South end of the Special Tree NeuroCare Center is our 15-bed residential wing, NCC South. Adjacent to our north subacute wing, the residential program at NCC South provides a perfect transition from our acute care with access to all of our campus amenities and staff. NCC South has its own visitor entrance, common areas, and dining room but benefits from our on-site therapy clinic, dietary, laundry, and housekeeping services, and access to our interdisciplinary medical and rehabilitation team.

Heavlin Residence

Built from the ground up especially to serve the needs of Special Tree patients, the Heavlin Residence is a transitional or long term living environment within the Special Tree NeuroCare Campus. Residents enjoy increased independence in barrier-free home modified to accommodate the needs of persons with disabilities while maintaining a warm, family-like home setting. The Heavlin Residence includes six private rooms, kitchen, living, and dining common areas, a three season portch, and close access to the therapy rehabilitation clinic and other NeuroCare Center resources.

In addition to our full service therapy clinic, on-site amenities include a walking path and nature trail following a perimeter of wetlands and wildlife habitat, pond with gazebo and accessible fishing area, greenhouse, indoor pool, and more.
